Preparation for medical consultations and procedures will be made upon receipt of reading the General Risks of Surgery and returning the "Quote Form", and reading and accepting the conditions of the Disclaimer. The scheduled service fee charged by Advanced Medi -Travel Cosmetic Consultants Pty Ltd to organize your bookings is $365.00 including GST.

A $100.00 non refundable booking deposit is required to be paid to commence medical arrangements. This deposit is paid to Advanced Medi -Travel Cosmetic Consultants Pty Ltd, and later deducted from the final payment of $365.00 after confirming consultation and surgery schedule. Payment of $365.00 including GST can be made via cheque, Eftpos, or direct credit into Advanced Medi -Travel account. The complete $365.00 needs to be paid to secure your medical consultations and theatre booking. After the $365.00 is received, your arrangements will then be finalized, as per your instructions. Advanced Medi -Travel's fee of $365.00 is non refundable once confirmation of surgery is received and the money is receipted. All travel and transfer postponement and cancellations must be notified 30 days prior to travel. This is an agreement between you and the respective companies only. Final costs for any medical procedures requested will be discussed on pre surgery consultation between the two parties (surgeon and client) and paid by the client directly to the hospital after consultation. It is at the sole discretion of the client to proceed with any surgery procedures after final costs are provided by the surgeon. This is an agreement made solely between you and your surgeon. Costs may vary depending on any additional procedures, or changes in procedures that may occur whilst in Thailand. Including but not limited to currency changes. Advanced Medi -Travel accepts no liability, responsibility or input between any procedures and costs incurred between you and your surgeon. All estimated quotes provided by Advanced Medi -Travel are provided from the surgeon and hospital on the procedure requested by the client. The sole responsibility of acceptance of the procedures and final costs discussed upon your pre surgery consultation are solely between you and your surgeon. For this reason procedure prices quoted can be subject to change. When agreeing to proceed the Disclaimer must be thoroughly completed and returned to Advanced Medi -Travel. Failure to complete and return the disclaimer, Advanced Medi -Travel has the right to cancel all arrangements and services without notice or a refund.
